The ultra-spiffy wedding form

I am not currently using this form for technical (web-related) reasons. But I'm keeping the page up, mainly as a place-holder, in the hopes that I'll have a chance to figure out how to get it up and running sometime in 2009! (or 2011, as the case may be!)
